Please note that due to COVID-19 precautions, only one person will be allowed at a time.

Required documents for tax preparation:

• All 2020 tax forms (W-2, 1099, 1098-E, etc.)
• If you claimed unemployment benefits please have your 1099-G form. You can find it by visiting dol.ny.gov, logging into your account and searching for “1099-G”
• Photo IDs (including spouse and dependents)
• Social security and ITIN cards (including spouse and dependents)
• Direct Deposit information (account and routing number)
• List of expenses if you are self-employed
